At Grace Hearts AFH, our mission is to provide compassionate, faith-based care and supportive services to adults facing physical, cognitive, emotional, and social challenges. We are dedicated to creating a safe, nurturing, home like environment that promotes dignity, independence, and personal growth. We strive to empower residents through individualized care plans, life skills training, and community integration support-fostering hope, healing, and a renewed sense of purpose.

Yes. Grace Hearts AFH is enrolled with Wisconsin MCOs, and we also accept private pay.

How many residents live in the home?

Our home is licensed for up to 4 residents, allowing us to provide individualized, person-centered care.

What safety measures do you have in place?

We follow Wisconsin DHS regulations, conduct regular fire drills, have emergency plans posted, and ensure the home is ADA-compliant and secure.

How do I start the admission process?

You can contact us directly or ask your case manager/discharge planner for a referral. We’ll schedule a tour, review your loved one’s needs, and discuss placement options.

At Grace Hearts AFH, we believe every person deserves more than care they deserve compassion, dignity, and a true sense of belonging. Located in Milwaukee, Wisconsin, our Adult Family Home provides a safe, supportive, and faith-based environment for adults who are aging, living with disabilities, or seeking a community where they can thrive. We are licensed through the State of Wisconsin and follow all DHS Chapter 88 regulations to ensure the highest standards of care and safety. But beyond meeting requirements, we go further creating a home where residents are treated like family, not just clients.
